Procedures such as Colposcopy and LLETZ treatment for abnormal cervical smear, Vulvoscopy and vulval biopsy for lesions, Cervical polyp or vulval cysts removal, Hysteroscopy for investigation of bleeding and removal of uterine polyps, fibroids, taking endometrial biopsy, as well as Ultrasound scans and blood tests can be done all in one appointment. Lower Genital Tract Diseases

Vulvodynia (chronic vulvar pain without identifiable cause) and vaginismus (involuntary pelvic muscle spasm preventing penetration) require a thorough assessment, including medical history, physical examination, and exclusion of infections (e.g., candidiasis, herpes) or dermatological conditions. For vulvodynia, a cotton-swab test helps localise pain, while vaginismus is diagnosed clinically, often with observed muscle tightness during attempted examination.

Management involves a multidisciplinary approach: vulvodynia treatment may include topical lidocaine, hormonal therapy, neuropathic agents (e.g., amitriptyline, gabapentin), pelvic floor physiotherapy, and cognitive-behavioural therapy (CBT). Vaginismus is primarily treated with progressive desensitization, pelvic floor relaxation exercises, vaginal dilators, and psychotherapy to address anxiety or trauma. Patient education, reassurance, and a tailored approach improve outcomes, with referral to specialists (e.g., pain specialists, or psychologists, or psychosexual counsellors ) when needed.

Pre-Cancer Screening and Menopause

Hormone replacement therapy (HRT) remains the most effective treatment for moderate to severe vasomotor and urogenital symptoms, with individualised regimes (transdermal/systemic oestrogen +_ progesterone) minimising risk (thromboembolism, breast cancer). Local oestrogen therapy is preferred for isolated vaginal trophy. Patient education on long-term health (cardiovascular, bone, and cognitive health) and regular follow-ups ensure safe, evidence-based care, while alternative therapies (phytoestrogen, cognitive behavioural therapy) cater to those avoiding hormones.
